We can now use a new online service called Register with a GP Surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. If you do use this method please also download the patient health questionnaire practice questionnaire and email to the surgery or bring with you when you attend. To register for online services we will need to see your proof of identity.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Although we are able to register out of area patients, there are problems associated with this in as much as if you need a home visit then we will be unable to provide it ourselves. In this case call 111.
